HealthMatters Program Application (Winter 2026)

Application Instructions

IMPORTANT: ONE Application per Organization only.

Application deadline is January 9th, 2026 end of business day. Approved Organizations will be contacted by January 16th, 2026.

Up to 15 organizations may participate. These include, but are not limited to the following:
1. Community based organizations who provide services to people with intellectual and developmental disabilities (day and residential programs)
2. Project SEARCH sites
3. Schools
4. Managed Care Organizations

This program is one (1) year long and requires the following commitments:

1. Attend one 3-hour webinar (minimum 3 support staff participants)

Option 1 | Wednesday, February 11th, 2026 (1pm-4pm CT)
Option 2 | Thursday, February 12th, 2026 (10am-1pm CT)

2. Complete asynchronous training activities and teach Lesson 1.

3. Run at least one Virtual Coach: 12-week HealthMatters Program.
